Applications for Special Service Provider Certificates

• INITIAL SPECIAL SERVICES CERTIFICATION (TYPE C) - pdf
  For first-time applicants or applicants whose previously held 5-year special services (Type C) certificate has been expired longer than one year. This application includes options for the Regular, Provisional, Temporary, and Reemployment special services certificates.


• REGULAR CERTIFICATION - pdf
  For holders of a current Type C Provisional, Temporary, or Reemployment certificates who have met all requirements for the Regular 5-year certificate. This application does not apply to teaching certificates, only to special services (Type C) certificates.


• ADVANCED CERTIFICATION APPLICATION (TYPE C) - pdf
  For holders of a current 5-year Type C certificate who also hold National Board Certification and wish to apply for the Advanced Certificate, as well as for current Advanced certificate holders wishing to renew their Advanced Type C certificate.
